You have been approved for an enrollment. When only the company name is shown, that means you have been approved for a free phone and service from that carrier. If you see SIM ONLY, this means that the carrier will provide you with service/SIM card but you must purchase a phone or bring an fully unlocked phone to get your service started.
We sell a variety of unlocked phones in our store for your convenience.
National Verifier or the Carrier has requested documentation;
1. Proof of Birthdate: Valid ID, US Birth Certificate, US Passport, I-94, etc.
2. Proof of SSN: SS Card, Last years tax return, etc.
3. Proof of Address: Current Lease, Utility Bill, Letters from Government Programs, etc.
3. Proof of Eligibility: EBT Award Letter, Medicaid Approval Letter, Last years tax return, etc.
Unfortunately, the carrier listed has declined to enroll you in their program or an error has occured.
*At the time of this post, we have been informed that the lifeline program currently has a 4 enrollment limit per 12 month period. If this applies to you, we highly recommend contacting your current carrier to inquire about phone/SIM card replacement options.
